I hereby unconditionally and irrevocably consent and permit the above-named Participant(s) to fully participate in all activities at Sunway Sanctuary fully recognising that there are risks known and unknown, foreseeable, and unforeseeable involved in participating in these or similar activities. I accept that Focus on the Family Malaysia has taken reasonable and prudent steps to reduce known and foreseeable risks. Knowing these risks, I agree that participation in any of the activity is entirely voluntary.

I understand and agree that neither Focus on the Family Malaysia nor its directors, employees, sponsors, volunteers or representatives shall be held liable in any way whatsoever for any risk or liability howsoever arising, which shall include, without limitation, injury, harm, damage or death or other consequential losses to life, limb or property which may occur to the above named Participant(s) as a result of participating in these activities and hereby release, save and hold harmless Focus on the Family Malaysia against all risk or liability sustained whether directly or indirectly due to participation in any of these activities.

Further, I do consent to any and all medical treatment that may deemed necessary for the Participant(s) should he/she require such assistance, including the ordering and administering of medications. I agree to follow Focus on the Family Malaysia to transport Participant(s) as needed and to use a photocopy of this form as my authorisation when necessary.

Focus on the Family may use the Participant(s)' photos, digital images, video, and sound recordings in future promotional materials. I have read and voluntarily agree to the statements herein.
